The Joint Commission on Accreditation of Healthcare Organizations has identified fires related to oxygen in the homes as the number one cause of Sentinel Events in the Home Care setting.The Joint Commission has developed two Sentinel Event Alerts on this topic and National Patient Safety Goal Number 15 has been developed as a result of the need to assure that home care organizations are doing all that they can to reduce risks through proper assessment of risks in the home setting, educating patients and families on measures to mitigate risks and to communicate with associated physicians, home medical equipment supplies, and the community related to risks.

UPMC Home Healthcare has identified opportunities for improvement in this area related to monitoring of patient incidents and near misses.This proposal focuses on developing an evidence based simulation exercise that could be utilized in orientation and/or continuing education to teach new clinical staff the proper assessment skills related to oxygen safety and management in the home setting.Additionally, it would address best practices in educating patients/families related to oxygen safety and management, appropriate documentation and communication related to issues/risks.Resources for management staff would also be shared.
